Gracepoint Wellness Adult Case Management Program- Tampa

Gracepointís targeted case management program provides goal-oriented and individualized support to individuals 18 years and older. Assessments include emotional, social, behavioral, and developmental functioning within the home, work, and community. Based on this assessment, an individualized service plan will be developed, link each person with community providers or resources, and monitor the goals weíve established together.
